All sampling and analysis must be approved before a sample can be removed or any testing can begin. All testing is subject to the conditions listed in this form. For reference, our Material Sampling Procedures document outlines the sampling procedures, and we publish a list of test types approved for sampling.

If you are on site, notify the CRC Service Desk when the form is completed and ready for review. If you are not at the CRC in person, you may submit your form and your request will be reviewed before any approval will be granted. A CRC staff member will contact you to finalize the sampling request.
